If traveling by yourself, leave blank, a parent accompanied by a child should list the child (but they too must have a completed application), students traveling with a parent should just mark through the word "Spouse" on a line and write Mother or Father and the other information that is requested

• 13. If this is not your first trip to Uganda, list entrance date(s) of all previous trips. Otherwise, leave blank.
